82 year old male, heavy smoker, developed right heel ulcer during rehab after a femur fracture. No pulses in the foot but they could "heard" with doppler. Told to stay off their feet until the ulcer healed. CT angiography shows severe multilevel peripheral artery disease. A simple inflow endovascular procedure restored enough blood flow to achieve healing.
Recurrent lower extermity DVT should prompt an investigation for venous outflow obstruction. Too often these patients are under treated.
